About the company:
This trusted and well-known Community Home Care/Aged Care organisation provides outstanding dementia services to the Aged community situated throughout Brisbane with support from the National Head Office that focus on delivering high-quality support to all their staff and home care clients.

About the role:
The Case Manager will be responsible for the provision and co-ordination of dementia care standards, compliance and education programs that meets the individual needs of the clients and providing support and advice to the care staff team, clients and families.

  • Demonstrate comprehensive knowledge of the Community Home/Aged Care sector, including best practice dementia care standards and processes.
  • Coordinate, educate and mentor the care staff with dementia specific training
  • You will advise and support clients across community home care locations, reviewed dementia care plans, policy and procedures that enhanced the skills and capabilities within the care staff to achieve quality dementia outcomes for clients and families.
Skills and experience:
  • Experience Allied Health or Nursing professionals
  • Knowledge of Dementia
  • Experience in working closely with multidisciplinary teams
  • Quality Training included with fantastic support from management
